Job Title: Home Health Registered Nurse RN Full TimeJob Summary:Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Registered Nurse RN Full Time. As a Home Health Registered Nurse, you will provide high-quality, patient-centered care to individuals in their homes, working collaboratively with our interdisciplinary team to develop and implement personalized care plans.Responsibilities:

• Develop and implement individualized care plans in accordance with physician orders and client needs.
• Provide education and support to clients and their families to promote optimal health outcomes.
• Collaborate with primary caregivers to ensure seamless care transitions and optimal functioning levels for each client.
• Coordinate with the client's physician and other caregivers to ensure timely and effective care.
• Provide a range of skilled nursing services, including phone triage, patient education, observation/assessment, wound care, infusions, catheter care, and PICC line dressing changes.Requirements:
  • Active RN license in the state of application.
  • Valid CPR and driver's license.
  • Medicare skilled nursing experience preferred.
  • Basic understanding of Oasis.
